These Terms and Conditions ("Terms") govern your access to and use of services provided by Rapid Revive ("we," "us," or "our"), including mobile IV hydration, executive consulting, staff training, life coaching, and patient advocacy services delivered across Valley-Wide Arizona. By booking a service or using this website, you agree to these Terms.

1. Services

All IV and clinical services are administered by a licensed registered nurse (RN) under the standing orders of a collaborating physician. Services are elective and supplemental to — not a replacement for — ongoing medical care from your primary physician.

2. Medical Eligibility

You must be at least 18 years of age and complete a health intake before receiving IV therapy. We reserve the right to decline service for safety reasons, including but not limited to: significant cardiac, kidney, or liver disease; pregnancy without physician clearance; active infection; or intoxication beyond our scope of practice.

3. Booking, Payment, and Cancellation

• Full payment is collected at booking unless otherwise agreed in writing.
• Cancellations made more than 24 hours before the appointment receive a full refund.
• Cancellations within 24 hours may be charged a fee equal to 50% of the service to cover supplies and reserved nurse time.
• No-shows are non-refundable.
• If we cancel due to nurse unavailability, weather, or safety concerns, you receive a full refund or reschedule at no charge.

4. Travel & Service Area

We serve the Phoenix Valley including Scottsdale, Phoenix, Paradise Valley, Tempe, Mesa, Chandler, Gilbert, and Queen Creek. Locations outside this area may incur a travel fee disclosed before booking.

5. Assumption of Risk

IV therapy carries inherent risks including but not limited to bruising, infiltration, infection, vein irritation, and rare allergic reactions. By accepting service, you acknowledge these risks and confirm that the nurse has reviewed them with you.

6. No Doctor-Patient Relationship via Website

Information on this website is for general educational purposes only and does not create a doctor-patient or nurse-patient relationship. Always consult a qualified healthcare provider for medical advice.

7. Intellectual Property

All website content, branding, and materials are owned by Rapid Revive and may not be copied or redistributed without written permission.

8. Limitation of Liability

To the maximum extent permitted by law, Rapid Revive's total liability for any claim arising out of services or website use is limited to the amount you paid for the specific service in question.

9. Governing Law

These Terms are governed by the laws of the State of Arizona. Any disputes shall be resolved in the state or federal courts located in Maricopa County, Arizona.

10. Changes

We may update these Terms at any time. Continued use of our services after updates constitutes acceptance of the revised Terms.
